Description: This script allows you to create a form on your web site. When visitors fill it in, their responses are emailed to you. You can include as many fields as you like in the form, and each message sent to you also includes the visitor's IP address. You can use this form as an alternative to publishing your email address, and thus avoid your email address being harvested by spambots.
