Description: Very easy to use but fully functional free contact from that will not allow submissions unless all the fields are filled in correctly.
2 files. Edit the page title and subject options in one and insert your email address and a back/home link in the other. Also included is a nice email sent icon.
Can easily be used for other submissions like user reviews just by changing subject options.
