Very secure. Combats contact form spam. Very easy to set up, just one file. The minimum you need to do is add your email address. Instructions in plain English. Free, friendly support. Comes with a basic contact form and can process any form or additional fields. Set required fields. Attach file uploads. Upload files to your server. Block or allow file types. Support for reCAPTCHA. CSV output to attachment or file. Autoresponder. Attach a file to the autoresponse. Set encoding, utf-8 etc. See sender's IP address. Block email headers. Block IPs. Block web addresses. Form Tools is a form processor, storage and data access script written in PHP and MySQL, designed to work with any existing web form. With a few minor changes to your form, you can stop using old-fashioned form-mail scripts and instead store form submissions in a database, instantly providing you and your clients with options such as mass data export via Excel, printer-friendly pages, data searching and sorting, form submission editing and optional email of form submissions to administrators and users. You can even add filters to each client account to limit what submissions they see based on any criteria from your form.

Forms Expert is a great, easy-to-use tool to create powerful form processing scripts. You can use those scripts for different purposes, such as contact or feedback forms, surveys, CV form, price or catalogue request form, etc. You can setup your forms to validate inputted data: set mandatory fields, set email address syntax validation, etc. Also you can create "Preview" and "Thank you!" ("Success!") pages, so your visitors could preview inputted data and be sure that their submission was successful.
Setup your processing scripts to send form data to you and your visitors, save it to MySQL database or flat text files. You can send unlimited number of emails for your forms. Forms Expert has step-by-step wizard, so you can setup your forms in a few clicks. No programming is required! Form setup would be easy even for newbie.

Mailcode is a simple feedback form system, designed with the novice web designer in mind. It utilizes CAPTCHA technology to prevent spam

HiQFMail is a PHP form handler designed with security and flexibility in mind. Configuration is performed outside the form and normally no modification of the PHP source files is required.
Installation is as simple as download, extract source files, edit the configuration files, upload to your server and use. Some Features:
Multiple attachments, Single or Multiple selection from check boxes or lists, Limit types of attachments allowed, Block attachments by extension, Multiple recipients, Select a subset of recipients from a default list, Auto responder and more. PHP4 & PHP5.
While not formally supported some have had success using on Windows.
