Thank you for your thoughts and the link. I will look over the info you provided. Although I don't think I am mentally capable of doing this myself at this time, perhaps I can pick up some knowledge/ideas from your tutorial.

What you are describing can be done, but definitely needs javascript/jQuery if you want a "modal contact form" to pop up when they click the link. jQuery has an awesome dialog widget that can easily be used. As far as making their info pop up inside the form, I actually posted a small tutorial on a way I figured out how to do it on my site: jQuery, PHP, and Forms
