I need the following which can be distributed to my clients. t must be setup in the following ways:

[1] It must use SMTP where the client will have to enter their mail servers, e-mail address, password, choice of port to be used (25 or 26). It can not use sendmail. I have to many issues with scripts relaying e-mails through our servers and sent as nobody@servername.com.

[2] It must be customizable, meaning it must have a header, footer and css so background colors can be changed.

[3] It must be able to be added to or taken away from. For example one client may just want name of person, e-mail address and text box. Another client may need to add sections to the form for their visitors to add or be asked certain items such as their location, choices of 2 or 3 items using radio buttons.

[4] It must be able to work with register_globals=off


Payment for work will be made with paypal. Price for job should be posted here. Please do not send me a PM with the price, all offers must be posted here. Payment is given upon testing script on our servers and viewing code to insure the above is what I asked for.

Thank you but that uses sendmail and not SMTP which is what I do not want.

Try contact them. I worked with them in past and they can customize PHPForms.
