URGENT help needed $$$ - Formmail script

I am looking for someone to setup a formmail script to work on a site hosted on Windows 2003/Plesk 7.

I have tried to use formmail by Matt's script archive, ported to Windows version, however was unable to get this working.

The script can either be a modified version of formmail.pl to work on Windows, or could be a PHP script, however it must be compatible with standard web forms.

I do not require any complex programming, just the ability to collect fields from web forms and output these to email.
