RE: PHP form with required fields, renamed to "download-form.htm"

I sure could use some help on this since I am a dummy at any scripts.

I did find a nice free script like I need at http://www.warkensoft.com/scripts/form_contact.html
This was just what I wanted with the required fields but I did not know how to change it for my form fields.
I am not a Script expert in knowing anything beyond a few simple changes.
I just felt that I could not edit this script to be like my download-form.

I have created the form layout as I need it. When submitted, it redirects them to a thankyou.htm page.
I just need someone to insert the required field scripts and a download-form.php
