PHP Programmer - Form

I have been working on a new website for quite some time. I have the template and most sections of the site already completed. Basically, i'm making a tutorial website for Alias wavefront maya and need help with the submit form.

Basically, i wan't the form to contain these fields:

Title of Tutorial :


Website Name :

Website Url:

Tutorial Url 

Categorydrop down menu

Tutorial Avatar Url 
//Browse Button/upload 100x100

Description 
I want the form to write all data to a mysql table. Here is my table structure

For the form itself, I want the user to have to upload a preview image/avatar using a browse option in the form. I want the form to check and see whether or not the image to be uploaded is 100x100(jpg, gif, or png) then upload the image to a folder(tutorial/avatars/). Then I want the url to the image saved in the mysql database under 'avatarurl.'

I'm looking to pay someone $10 usd to code this simple form in php. If you have any questions, let me know. I'll pay via paypal.

$10 is a little cheap.. $25 and I'd do it no worries

LOL yeah its really irritating how people post things up like "I will offer $10 for this simple PHP form" as though they know the market value for this kind of work or something.

I can't rememeber the last time I was actually paid what a job is worth.. you quote a fair price of $300 for like a week's work, and some 'offshore' development company offers to do it for $25 or $3/hr.. should be some kind of official award rate that a developer can reference and say "this is indsutry standard pricing"..

Then you'll get a job and the guy will say he'll you what its actually worth, and your thinking he has some common sense, but then he turns around and rips you off...*sigh*

It is fairly hard to get a decent programming job though. I'm a full time PHP programmer and I like to do some on the side things for extra cash, but $10 here and $15 there isn't what I call "extra cash".

Yeah exactly it all gets brought down to the lowest common denominator, The thing is I cant imagine the majority of these foreign companys being any good.

I think they could be, its just many of them know that what they are getting paid is cheap for western standards so they can get away with doing a bad job. They just have an economic advantage in that the prices they charge are cheap for us, but its about averge, or even higher than average for them. It just sucks for us...
