Ok, firstly, thanks for your interest, what I can do is give the form, i.e the fields required to write to mysql, send, and call from mysql, if you write a template based on that information and the bit of code that connects to a database, with empty database, user, location, and password, fields, I can actually fill out the database details to connect (that is one thing in my small knowledge base that I can do, lol.)
I've put an example of the form I want to use on my friends site at

you will see the company name and the tax number (this field will be changed to ubi, every other field name will remain the same) at the top, these are the two fields that when entered will search the database for a match and fill out the rest of the form, down to placement information, which will remain different everytime.
By the way, I have no knowledge of php other than changing the fields in the connect part of a piece of code. as well as the template for the form I will need the full MySQL command/or install file to create the tables, fields etc inside the database, sorry I'm so slow, lol.
aswell as interact with the database, I want A secure method of emailing the form to an email account. and emailing a generic receipt to the form senders email account.
Is all of this possible from one form, securely?
