PHP/MySQL Custom script needed, Willing to pay small sum

Hi people,

What I'm looking for is someone to write me a script (PHP/MySQL) that not only checks the fields in a form, sends an email with the data entered and writes the data to a mysql database, but I also want it to check the database when the first two fields (company name and tax no) are entered by the user to see if the information matching the company details is already in the database and fill out the rest of the form for them, minus the part that differs with every request (the actual order.)

I have been told this should be easy (to someone who knows how, which doesn't include me, lol,)and we are a very small, new company, with a very tight budget so I am only willing to pay a small sum, my first offer will be around $30 us, if this is not enough I may be able to increase the offer, as we are desperate for help.

Do you need a template with it, or just very basic, very simple layout with __JUST__ the functionality?

Ok, firstly, thanks for your interest, what I can do is give the form, i.e the fields required to write to mysql, send, and call from mysql, if you write a template based on that information and the bit of code that connects to a database, with empty database, user, location, and password, fields, I can actually fill out the database details to connect (that is one thing in my small knowledge base that I can do, lol.)

I've put an example of the form I want to use on my friends site at http://www.ptassassins.com/keysdesigns/

you will see the company name and the tax number (this field will be changed to ubi, every other field name will remain the same) at the top, these are the two fields that when entered will search the database for a match and fill out the rest of the form, down to placement information, which will remain different everytime.

By the way, I have no knowledge of php other than changing the fields in the connect part of a piece of code. as well as the template for the form I will need the full MySQL command/or install file to create the tables, fields etc inside the database, sorry I'm so slow, lol.

aswell as interact with the database, I want A secure method of emailing the form to an email account. and emailing a generic receipt to the form senders email account.

Is all of this possible from one form, securely?
