SQL or ACCESS? DB Needed

I am looking to create a pretty simple or basic database. Some fields will also be shown in 2 areas, not just one. Here is a PDF of the type of information and rough layout or ideas. Just curious...... Should it be done in SQL or ACCESS or ??

Requirements:

Access from internet VIA login name and password (multiple users)

Ability to edit configuration and move to another server if needed over time.

Ability to easily add field if necessary.

Majority of fields are basic data entry fields but some are drop down menus or check boxes.

Would like for it to have a "memory" of previously entered information to make entry faster.
