Hi Drunken Perl Coder,
Because, Web site (where this form should be displayed) has already developed HTML pages. So, input and output into the Perl script should be from the static HTML page(s) which are already in use.

I see that this is hard. That is why I am asking about help.
Help, help. Please, help me.

My advice: tell the site owners that using the existing html page is OK for the initial form (form1) but after that the pages should be dynamically written by the the perl script and last only as long as the user has their browser window open. They/you are trying to fit a round peg in a square hole.

Not really. He can create a static html page on the fly by writing the results "stdout" to an html page. The visitor will not be the wiser as all they will see is another form or information on a static page. In a sense, this has been a workaround for PHP's capability for years, but honestly, the best overall route is to use session techniques with either MYSQL or flat-file databases. Let's try to give the OP what he asks for.
