I've seen so many guestbooks like this one, in terms of the functions, and you can find the one you want at http://www.hotscripts.com/ or somewhere like that. In terms of the design, I guess this site owner did some modifications to it so that it fits his/her site's overall design, so if you are talking about the deisgn, you need to customize it by yourself.
