What to watch when implementing an upload script

Hi,

I am trying to add an upload avatar function for a forum script that I am trying to finish and I was wondering what is the best approach to do this for the most secure way?

Should just create a folder CHMOD it 777 and let the users upload it there or straight enter the image file as a binary(Blob) to MySQL?

Which one would be safer and headache free.
