create form to update config.xml file

Hello, I'm hoping someone can here help me. I'm somewhat new to programing and not sure how to create a form that can use to edit settings in a config.xml file. I've looked all over for some sort of code I could use that would do this but I've had no luck. The settings I want to be editable are in red.
