Help needed on .htacess mod rewrite

Hello guys can any one help me in this issue i want to rewrite my urls can u tell me how can i rename http://mysite.com/Led/index.php?d=java//2012_ap to http://mysite.com/Led/java/2012-ap.php or plese help me
please give me a modrewrite code for this
