Question Conference signup script

Hi
I need to find a script that will help me handle sing-ups for a conference. I want to display a couple of input-boxes and take an e-mail address, name and organisation name from people. The data should be stored in an SQL database and be easily retrieveable, an option to export as .csv for example. (I need to copy the e-mail adresses to Outlook to send mails since the server can’t handle the sending of mails directly) Please don’t suggest PHPlist , I can’t get it to work .

I’m currently testing Webinsta mailing list , but it doesn’t have a box where people can enter their organisation name.

It is a very precise requirement, and I haven´t seen something like that before. That is why we finally made something custom for us.
Check this site we made for an example:

You may try putting a request for quote in any of the freelance bidding sites
available out there.

Thanks for your answer. I've actually started using PHP-Surveyor and that pretty much fits my requirements.
