Hi, Im looking for a script for an "Angler program"

Basically a simple form with some questions (about 10) of the clients Daily Catch, where you will be able to upload your pic....

Once you hit on Submit, you will see your posting plus the thumbnail of the image you just uploaded...

thats it...
Im sure its simple.. It would be like a guestbook type of thing... theres actually a guestbook we have from: http://proxy2.de/guestbook/
If you can customize that same guestbook with just adding our questions... that would be great too.

Just need to know from you Masters how long will it take, how Much $, and where do I pay you...
