Hi,

I need a script were visitors of my site can fill in their name, some details and pick from a list a year and a dancecompany they have performed with. If they fill this is it would be nice if there was a searchoption that can seach and list by name, year and dancecompany.

Does anyone know something like this of can someone develop it for me ?
