Online Form Emailed to Display on Web Page

I am looking to be able to have an online form submitted to and email address and at the same time submitted to be displayed on a web page. The web page would display a maximum of 50 responses at any time. The older responses would be replaced by the newer ones as they are submitted. No vetting is required but would need the ability to delete responses if necessary. Prefer PHP if possible.

Yes I am willing to pay. Cost will determine whether the project goes ahead.

I have over 2 years of real world experiance in PHP/MySQL programming. I can also use flat file dbs, it's not a problem.
