Image Gallery + User Fields

I'm looking for a database script where a user can input...

Field 1
Field 2
Field 3

and the have the option to upload an image(s) of the product, device they're adding to the database..

Then will show a list of those product/device in an alphabetically list...

ProductA Field 1 Field 2 Field3 Field 4 Image (Thumbnail)
ProductB Field 1 Field 2 Field3 Field 4 Image (Thumbnail)
ProductC Field 1 Field 2 Field3 Field 4 Image (Thumbnail)
ProductD Field 1 Field 2 Field3 Field 4 Image (Thumbnail)
ProductE Field 1 Field 2 Field3 Field 4 Image (Thumbnail)

Kind of like an database & image gallery, but in a tabular format, listed/sortbale by the field1, 2, 3.

Anyone seen something like this before?
