Sorry - just found a scripts request section to the forum

I am an exercise physiologist and a wellness coach. I am building a website to manage the clients I coach. My PHP skills are limited, but I am able to modify script if it is not too complex. I am looking for some existing PHP script to manage my clients.
To give yiou a sense of the workload, my goal is to take on two new clients per week. I would like to gather information on each client, including name, address and a whole lot of health and fitness information on separate screens.
Here is the process that I would like to follow:
> assign a UN and PW to the new client to give them access to the info forms
> after logging in they go to a table of contents page to pick one of about six choices of pages to go to
> the first choice is demographic info, name, address, phone, height, weight, etc.
> the second choice would likely be health and fitness history
> the third choice might be questions about their barriers to good health and fitness habits
> and on from there up to about six forms
None of this information needs to be processed in anyway, other than stored to a database for easy access by both me and the client. I don't want anyone else to have access to these accounts. Each form should have a print mode, that would not include any of the webpage graphics, but that is easy I could add that if it is not part of the template.
This seems pretty easy, but still beyond my skills. I'd hire a programmer, but my budget is tight. Modifiable template script seems to be the answer. contact management script is close, but doesn't have the individual password protection.
Does anyone know if this exists as template script?
