MySQL database (1 table) and PHP search

Hi there,

I'm looking for a simple MySql database (only one table with two fields) and a PHP form that searches this MySql database, and shows the results in that page.

- Programming Language
PHP page with search field to scan a MySQL database with only 1 table.

- Platform
Sorry? Our Internet Service Provider says PHP/MySql works on their servers.

- Free or Paid
Willing to pay for a working example.

- Type of License
Open license?
