Database script.

Hi guys.
I need some help creating a script and i know you can help me.

I want a basic form on my website where a person would input a 15 digit number and click send.
I then need the script to log into a database (i dont have the data base address, but i have access to it via my own username and password, which i access via a private website), and input that number into a field to validate the number.
This then returns a response confirming the number, or not, and i wish to publish this result on my site.

Is this possible?
