php script needed for uploader

Currently have a website and server where our techs upload QC photos daily. Need php script written so that tech can select from a form first OFFICE, TECH# DATE. This information would determine which directory the photos would go. Also need to have the script resize the photos.

Multifile upload needed where they can highlite all the photos in the batch.

Let me know what additional information would be needed for you.
