Talking post fields to a text file?

hi... i was searching online for a php form script that can do the following... i really need this... if anyone can help... thanks a lot!!!

write the 10 fields with a comma separate them to a text file... and each time could be time stamped... and list each line for a custom listing category

mainly its just for a contact listing posting

i want the customers come to the store being surveyed to have their contact information saved.... but don't want to fill these contact book software which have all these different field on different page which makes it very annoying...

composition of the script should look something like this
[ field 1 ],[ field 2 ], [ field 3 ],... [ field 10 ], registered on [ date ], for the purpose of [ newsletter ]

when the second person or second post... the new post will go to the next line...

if anyone can help... thanks in advance... maybe someoneelse is in need of it too...
