Hello, Sorry about the long title.

I need a contact form for my health isnurance website. On this form vistors will be able to fill ou their vital info and be able to add any family memebers.

Instead of having a form that show's a field for each extra contact I want them to be able to click a button that says something like "add spouse" and or add dependent" once that button is clicked their is now a field in the form for that family memebers info to be inputted.

I'd show you an example of what I'm talking about but I don't want to get labeled as someone trying to sell insurance here! lol.
