Script for populating a renewal letter from database credentials

I'm trying to figure out how to even start to approach this...

I'm trying to locate something similar to demonstrate some of these functions. I need to populate renewal letters with information from a database after expiration. Any tips, suggestions, etc?

What kind of database? MySQL?

What kind of server? Linux/Apache, Windows/Apache, Windows/IIS?

Available server-side scripting? ASP, PHP, Perl?

Existing language or application? Custom, commercial, none?
