May i ask help for a html script with java to copy to clipboard 3-5 form inputs.

Issue: _____________
Resolution: _____________
so result after copy to clipboard button is clicked is:
Name: Mr.X
Issue: has a broken wheel
Resolution: advised customer to get a new one. say good bye to old wheel.

can i request for a full code for this with reset button along with everything.
i will be working with IE browser only.
