Hi,

I am looking for a script where visitors to the website fill in forms or answer questions and after they have filled in the forms and answered the questions the script should generate a document which is based on the information from the visitor.

As an example this could be used for an application form - the visitor fills in his or her personal detail, answers questions which courses to attend etc and when all the questions are answered and the forms are filled in the visitor clicks the ‘ok-button’ the document is created and displayed on the screen, or even better, mailed to the visitor. The created document will contain a complete application form with for example logotype, the rules for applicants, guidelines etc.

Does anyone know of such script or anything similar to it?
