I need to create a form for an ROI calculator but have know idea how to write script!
Please help me!

There is nothing like calculator script. You know the how your form should look like. You can do all your calculations either in Javascript or in PHP.

That is the the problem I do not know PHP or JS, only HTML!

PHP is not that difficult. There are so many online resources that can help you to learn web programming languages online. One of such resource is hscripts.com. Do have a look. I am sure the online PHP Tutorial here can help you build that calculator.
