Wondering if someone can help connect me with a script that will support what I need. Essentially I need a really barebones (php) script that will allow me to track my employee availability, and allow them to log there availability online.

My knowledge is limited but I know HTML, and understand the basics of PHP so I can work with scripts and get them set up. But need help finding a script that works to my needs

Three Pages
1) Register for Shift
2) Current Availability
3) Admin Page (allowing deletion of entries, password would do)

PAGE - Register for Shift
This page allows the employee to indicate to us what day they are available to do overtime (basically they would put which days off they can work). So its allowing to sign up individual days - I dont want anything complex that allows them to block entire weeks, or to register every friday for the remainder of the year. If they want to book multiple days, they have to fill out the form once for each date available. The information boxes would be
1) Full Name
2) Date Available
3) Start Time Available (in 24 hour clock)
4) End Time Available (in 24hour clock)
5) Notes (perhaps a 250 character comment box, or something simple if they had any comments or restrictions on type of work etc)

All entries get stored to the my-sql database.

PAGE - Current Availability
Essentially just a horizontal listing of everyone's availability. With the top entry listed as the most future or most current date, descending down to the oldest date of availability by the calendar date (not so picky about further separating by individual times if multiple sign ups for a single day). So it would look like in a table obviously

Scott Jackson // DEC 31 2011 // 1900 // 0700 // I have access to a car to work any location
Bobby Franky // DEC 17 2011 // 1700 // 0800 // No restrictions
Andy Houston // DEC 01 2011 // 2000 // 1000 // Need company tool chest

PAGE - Admin Page
Same view at the Current Availability Page, just basically allowing the admin to delete old entries, or delete entries where we called the person and they aren't able to take the shift
