Exclamation I need a PHP Script !!!

I want the following PHP script :
1 - send a name with an html form to a PHP script,
3 - If the name does not exists in Database, add it to DB.
2 - If the name exists in Database, goto "error.html" (for example).
4 - I'm not sure regard text database or MySql Database, which one is better ?
I here any one that can help me ?
