Need User Profile Script!!!

Does anyone know where I can find a script similar to upoints @1 Personal Memoranda script? http://upoint.info/cgi/demo/memoranda/index.htm

I don't need any complicated features. Just a script that automatically generates a webpage from the image/text fields the user fills in.

Does anyone have any suggestions?
