I'm a mainly hand coding designer - well not much of a designer actually because I have no fancy skills with PS that enable me to create aesthetically stunning sites - I create things from scratch by CSS and HTML hand coding. So I'd say Notepad++ and MS Expression Web.

I rely heavily on ImageMagick to create gradients and modify creative commons licensed images from Welcome to Flickr - Photo Sharing.

If I can't get away with a gradient and someone else's images - I use PhotoShop.

Edit with vi/vim.

I started with the same, DW + photoshop 3 - today I just stick to photoshop and create all the html + coding by hand using 'edit plus' seems to work like a champ!

Me too, I'm combining Photoshop and HTML. I think this is the best option. You need HTML-skills anyway so why not doing it yourself? Dreamweaver's alright, too. Sometimes I also use Typo3 or Pirobase. However, with the combination of the Adobe-solutions you can be sure that it works properly.
