AFC Easy Form was created for the beginner programmer who would like to have a simple contact form on their website that will send the owner the persons name, email, phone number, referred by, contact type and a question / suggestion or feedback. The form includes email validation and will goto an error page should the form not be filled out properly. If the form is correctly filled out it will goto a Thank You page. The AFC Easy Form is easy to modify for your needs. All you need to do is add your email address into the output.php file and upload the files to your web hosting server. AFC Easy Form Includes all the files you need. You can edit anything you need to make this form customized to serve you better. There are no restrictions to this script so feel free to add or modify the code.
