AFC MySQL Contact Form
This Contact form is similar to our AFC Easy Contact Form in that it will send the owner the persons name, email, phone number, referred by, contact type and a question / suggestion or feedback. The form includes email validation and will goto an error page should the form not be filled out properly. If the form is correctly filled out it will goto a Thank You page. The main difference is that the AFC MySQL Contact Form will send an eMail Confirmation to the client and will store all the data into an MySQL database, plus it will provide you with a Table Display so you can view all the data that has been entered at anytime. The script includes instructions and is completely modifiable to suit your needs. There are no restriction and the code is open source. So feel free to add to it or change it as you please.

The free version has a totally encrypted script and the "thank you" page only goes back to them where you can buy the modifiable script for $19.95 (although there is a special offer atm for $4.95! how could I lose?). Here's the text from the page that confirms this: "You can easily modify the purchased version to your needs and redirect the form to your own thank you page." Basically unuseable if you want to control where your visitors go after they fill in the form, or if you want to build in verification, or if you want to change the fields that are on the form, or do anything at all with it other than what they have pre-programmed into it. Very disappointed. They're also up to v9.9 now. Perhaps this wasn't how it worked before in the earlier versions, but this is how it works now.
