All you need to do is add your email address into the output.php file in two places and upload the files to your web hosting server. AFC Simpli Easy Form Includes all the files you need. You can edit anything you need to make this form customized to serve you better. There are no restrictions to this script so feel free to add or modify the code to suit your needs
