FormPro lets you easily create sophisticated forms with a various number of field types (input line w/ optional email address verification, fields w/ optional max chars, radio buttons, checkboxes, select lists, multiselect lists, file upload fields). The form submits its data to an email specified in the config section of the script. Each field can be mandatory and then puts out predefined error messages if no value is specified/entered.

Excellent little php form script!

If you're looking for a feedback form, or a question form for your site that includes the ability to attach a file as well as a configurable captcha to avoid spam look no further. Easy to set up, easy to format the way you want, and as a bonus the resultant emails it sends are very nicely formatted (text or html) unlike most other form scripts you'll find out there. Add to this the great support from PlanetLuc and you'll quickly agree that it's a superb value for your seven Euros!
