Excellent Simple Script

I was able to quickly customize this script to intigrate it into a website I was working on. I was able to easily add fields and add them to the copied email and the actual email. My years of experience with PHP and Javascript are only a couple years and with a couple trial and error tests, I was able to figure it out.
The script works wonderfully!!
