phpESP is a great way to setup a differnt types of survey forms for your web site. You'll need some basic knowledge of PHP and MySQL, but once you configure it properly, you'll be able to create multiple surveys easily.
This script is ideal for sites that regularly conduct surveys (especially surveys with multiple questions at once)and want to keep an archive of past survey results.
In his "To Do List", the script author, James Flemer, promises to improve a lot of things, particularly the documentation that comes with the package (version 1.3 as of this writing). This improvement, IMHO, would give phpESP an even higher rating.
Overall, a very flexible, powerful, and highly customizable script. Highly recommended!

A very good program. Not hard to learn and an amazing amount of flexibility considering it is "free." I have used it for about 18 months.
