I love this application, but haven't actually used it commercially because it is designed to be used as-is. You can change the templates, but code changes are a no-no. That's understandable of course because of the unimaginable work that must have gone into it, but if you want to use this as an "open source" solution, you might be in for a challenge. Fortunately for the developer, there isn't anything out there that compares, so I thing this product should expect a good life span.

MAchForm has allowed us to have one single install for all forms used on our server complete with a slick online builder, CSS styles, AJAX, CAPTCHA and more. It's a great looking script with an easy install and wonderful support. Simply a great solution.
