Seems that the authors have changed to commercial model now. The script appends seamless to the design quality of Jax Formmailer (templates, multi languages, SPAM protection) and has an excellent manual.
BUT: there is no graphical interface to edit forms by WYSIWYG so that the user needs to learn a little HTML and to read the manual
