I'm currently seekings a survey script. I've come across dozens of them that offer all the features I need, but never incorporate user input details such as name, email, and phone number.
I am looking for a solution, for a small business, where they can send out an emal to a survey (which could be password protected, or requires an invitation code), which can be created from an easy-to-use administration panel.
Of coruse the script would need to have the functionalaity for different question types such as mutliple choice, textbox, radio buttons, etc etc.
So basically I need a solution for the following:
-Enter page is password protected (or invitation code)
-User detail entry (such as full name, email, phone number)
-Then have an administration panel to create mutiple surverys with multiple questions.
-Email the results to the admin, and to the person who filled out the survey.
-Keep track of who filled out the survey, alongw ith thier answers.
-Statistics
I have found numerous scripts that come close, but dont offer the user details input section, which I think it probabaly the most important.
