I'm a newcomer to the work of Scripts and have only had a website a short time...

I'm looking for a form with either a drop down box or radio buttons with say 4 categories to choose from so that when a user submits a comment, it refreshes the page with their comment under the heading they chose as their category..

For instance, NAME, EMAIL, URL, COMMENTS, REFFERRER and for referrer there would be Google, Friend, Choice 3, CHoice 4 and when they submit the form, their comments & info post under which referrer they chose...
