OFH is a form handler that can read all form types (including multipart/formdata for file uploads) and is able to send the data via mail. It also enables you to store the data in (a) file(s). The script is fully configurable - including language support. It has a built in autoresponder function and several security features. The new version comes with a simple browser for uploaded files, and many more configuration options, like the maximum number of responders.
