The Unit Command Climate and Assessment and Survey System is a PHP based survey maker that can be used to create any kind of survey or questionnaire for your site. The system allows for the creation of an unlimited number of surveys, questions, and answers. Surveys and results can be marked as public or private. Results are shown as bar graphs. The system includes the ability to hide or require questions based upon previous answers and can filter results based upon user answers to any multiple choice question.
