When I first loaded this at home, the sample forms worked fine once I scrolled past 500 lines of error messages which followed two familiar themes - assumed indexes (no single quote marks) and undefined indexes. In short to run this as is you need to turn your error messages off. Personally That drives me up the wall as I have my own code to debug too. This is an excellent piece of code apart from that and has saved me a lot of time. However, it took me 5 hours to clean up the code grammar :)
