DWsurvey is a PHP/MySQL/GD powered, automated, web based survey generation and analysis program. From a web based administration area you can create and manage an unlimited number of surveys and questions which can be grouped together and generate online reports and graphs in HTML, CSV, XML or PDF. In addition to surveys you can also create polls with instant public results. Single page, multipage, polls and logical surveys can be created and managed. All survey types can be embedded in existing webpages using AJAX or displayed standalone in the application itself. Question types supported, text, multiple choice, multi answer, ratings, date, date/time, matrix of questions. All questions can be divided into groups. Surveys can be restricted for login and an automated email link sent to those users without the need for manually entering login details.
