I've been spending a lot of time finding my way around the php world recently, assessing which scripts and classes can help me develop interactive sites without devoting my life to learning php from the ground up. This script is a gem. I needed to create an on-line survey page with 15 fields, storing the resulting data in a MySQL table. With this script, I had the job done in under an hour - preceded by some time spent exploring the download and experimenting with little trial forms. Like all php scripts, it would benefit from properly thought-through user documentation, but even commercial php products suffer from that weakness. Very highly recommended.

My life got a lot easier when I found this script! It's easy to install, and easy to use. The resulting form can be inserted into your page and the results are emailed to you, and stored in a database - either MySQL or flat file. On a scale of 1-10, this script gets a 20! Kudos to Musawir Ali!

After having suffered through downloading and installing many php scripts with many complex php files each requiring custom configuration and then only to find they still don't work it was refreshing to find this script that doesn't require any custom configuration at all! You just have to run the index.php file and a graphical interface walks you through creating any form you desire! Furhermore, this program can save form data into a MySQL dbase or into a simple text file, your choice! A very slick Admin page takes you right to your form data and displays in neat rows and columns. The program allows you to create basic to sophisticated forms all with a very professional appearance! Best of all is the support! Initially I had one little problem accessing the dbase. I contacted the author, Musawir Ali, who responded immediately with the solution. Thank you Musawir!
