We have a client that hosts w/ GoDaddy on a Windows server. GoDaddy's Windows hosting does not support PHP (or does only in safe mode), but this guestbook saved the day! Install was as easy as unzip and upload. Although I didn't really see an online install system or documentation, it's really self explanatory. The script uses a DSN-less connection to a simple Access database and is easily customized (header.asp/footer.asp). The default layout is clean as-is. GREAT JOB!
